Thank you for the troubleshooting steps!  Unfortunately, I have tried all of those to no avail.  Have you been able to successfully get NDI output to work past FCP version 11.0.1?   Do you work for NDI, and if so can you put me in touch with the developers of this plugin to help troubleshoot this issue?
